Your Support
The net proceeds of the Lottery will go towards the purchase of a portable ultrasound machine for the Rouge Valley Ajax and Pickering hospital.

Tickets
Tickets are $10 each.
Only 1,000 tickets will be sold. Tickets can be purchased between October 15th, 2009 and 10pm on November 21, 2009.
Payment Options
Tickets will be sold for cash only.
Terms & Conditions
Ticket buyers must be 18 years of age or older and residents of Ontario.
Adult ticket buyers cannot write the name of a person under 18 years of age on the ticket.
The name appearing on the ticket must be of a person 18 years of age or older.
In the event a winning ticket stub has more than one name on it, the RVHS Foundation shall award the prize to only one of the individuals identified on the ticket. The RVHS Foundation and Alcohol and Gaming Commission of Ontario are not responsible for any disputes that may arise between the different individuals whose names appear on the ticket.
All ticket sales are final. No refunds allowed.
Ticket purchases are not tax deductible.
Only tickets that have been paid for and/or have been verified as paid for are eligible to be in entered into the draw.
The following people are prohibited from purchasing a ticket:
- The Mayor of Pickering
- Members of the Mayor’s Gala Committee
- Members of the Boards of Directors of Rouge Valley Health System Foundation
Purchasing Deadline
Tickets for the draw can be purchased up to 10pm on the day of the draw, November 21, 2009.
Tickets are available from representatives of the Mayor’s Gala Committee
Tickets are being sold at the following three Scotiabank branches in Pickering:
· Pickering Town Centre – 1355 Kingston Road
· Brock & Plummer – 1020 Brock Road
· Hwy #2 & Whites Rd – 705 Kingston Road
Information on Ticket Stubs
Lottery participants must complete the ticket stub with their full name, address, phone number and email address.
Draws & Prizes
Draw Control
The Mayor’s Gala Committee is responsible for the ticket distribution and draw control.
There shall be a minimum of two Mayor’s Gala representatives present at the draw as witness to uphold the security of the Mayor’s Gala 50/50 lottery; however, any ticket holder is welcome to witness the draw.
The total value of the prize is half the total amount of number of tickets sold. The maximum prize amount is $5,000.
The draw will take place at the Mayor’s Gala on November 21st, 2009 at the Pickering Recreation Complex at 10:30pm.
Prize Claim
There will be only one prize winning ticket.
The prize winner is responsible for claiming their prize at the Rouge Valley Health System Foundation upon showing proper personal identification. Only the prize winner can claim their prize.
The Rouge Valley Health System Foundation will award the prize.
If the prize is left unclaimed 12 months after the original draw date, it will be returned to the Mayor’s Gala.
Winners
RVHS Foundation personnel will contact the winner by telephone, on Monday November 23, 2009.
The winner’s name will be posted at www.mayorsgala.com on Monday November 23, 2009.
Winners consent to the use of their names and photography by the licensee for publicity purposes.
Winners consent to their names being published immediately after the draw, and prior to them being personally contacted.
